Job summary

CarePartners in Asheville, NC is seeking a Registered Nurse to provide coordinated skilled nursing care in patients' homes, collaborating with the interdisciplinary team to meet goals and ensure high-quality care. You will perform home health visits, complete OASIS, support on-call rotations, supervise home health aides, and ensure documentation accuracy and communication with physicians.

The role requires an ADN/BSN, NC RN license, BLS, and at least one year of home health experience;

Qualifications

Associate's Degree or Bachelor of Science in Nursing.
Current RN licensure with North Carolina Board of Nursing (NCBON). eNLC license may apply; must obtain RN licensure within 30 days of start date.
BLS Healthcare Provider certification.
One year of home health experience.

Responsibilities

Assesses home care patients and identifies physical, psychosocial and environmental needs.
Completes OASIS and visit paperwork per agency policy; ensures accurate notes and communication with physicians and care team.
Communicates significant findings to Clinical Manager and physician and documents interventions.
Supervises home health aides and LVNs/LPNs to ensure quality and continuity of services.
Participates in on-call rotations and emergency calls per policy.
