Job summary

CarePartners Health Services in western North Carolina is seeking an experienced Registered Nurse to provide skilled nursing care in patients' homes, collaborating with an interdisciplinary team to meet patient goals.

You will complete OASIS and visit documentation, communicate findings to the clinical team, and supervise home health aides and LPNs/LVNs. On-call rotation and emergency coverage are required.

Qualifications

• RN licensure in North Carolina (NC BON); eNLC may apply and must be obtained within 30 days of start date.
• BLS Healthcare Provider certification
• One year home health experience

Responsibilities

• Assesses home care patients identifying physical, psychosocial and environmental needs.
• Completes OASIS, assessment and visit paperwork and documents coordinated care with physicians and team.
• Communicates significant findings to Clinical Manager and physician; documents interventions.
• Supervises and provides clinical direction to home health aides and LPNs/LVNs for quality and continuity.
• Participates in on-call rotation and emergency call per policy.

Education

Associate's Degree or Bachelor of Science in Nursing

Registered Nurse licensure in North Carolina (NC BON) / eNLC may apply
