Job summary

This full-time position offers a compensation range of $89,000 to $95,000 per year, a comprehensive benefits package, and opportunities for continuing education. In-office attendance is essential to achieve Enhabit objectives.

Qualifications

• Graduate of an approved school of professional nursing.
• Must maintain a valid clinical license per state requirements or compact nurse license.
• Experience as a licensed professional may be deferred with VP Operations or Regional President approval.
• Minimum of two years of nursing experience within the last 36 months.
• Minimum of one year of home health experience.
• Excellent knowledge of federal, state and local laws and regulatory guidelines governing home care.
• CPR certification is required.
• Valid state driver's license and automobile liability insurance.
• Strong technology skills, including mobile devices; MS Office proficiency.
• Preferred: prior management experience.
• Preferred: Homecare Homebase (HCHB) experience.
• Preferred: English/Spanish bilingual communication.
• Reliable transportation and ability to drive in all weather.
• Excellent oral and written communication and active listening.

Responsibilities

• Coordinate home health services to the patient per standards of practice and company procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visit.
• Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and maintain care coordination and continuity.
