Job summary

Anacalifornia seeks a Community Health Nurse to coordinate and provide skilled nursing care in patients' homes. You assess health needs, adjust care plans, and deliver treatments such as wound care, drain management, and catheter changes.

You will lead case conferences with a multidisciplinary team, educate patients and families on medications and home safety, maintain thorough documentation, and ensure safe, effective care while promoting wellness and independence.

Qualifications

• Current State of New Jersey nursing license.
• CPR certification valid.
• BSN preferred; graduate from an accredited School of Nursing.
• Strong clinical knowledge for home health care.
• Ability to coordinate care with multidisciplinary team.
• Proficient in documentation and attention to details.
• Valid driver's license and clean driving record.

Responsibilities
