Job summary

Jobtome in New Mexico is seeking a Registered Nurse for home health care. You will provide direct nursing care outside hospital settings, coordinate activities, and ensure policy adherence across neonatal to geriatric patients.

Responsibilities include assessing patients, coordinating services, administering medications, and ensuring safety and high-quality patient care. A sign-on bonus and relocation assistance may be available.

Qualifications

• Essential: 1 year directly related experience.
• CPR for Healthcare/BLS Prov or Prof Rescuers within 30 days.

Responsibilities

• Assess patient's physical, psychological, social and environmental status; develop patient care plan tailored to needs assessment.
• Provide nursing care and coordinates ancillary services to achieve treatment plan goals; supervises staff.
• Administer medications and treatments per protocols.
• Report observations and changes in patient status to team members.
• Provide follow-up to ensure problem resolution; record observations and care activities.
• Evaluate patient response and modify treatment plan as needed.
• Interpret policies to patients and families; utilize community resources for client benefit.
• Establish rapport with patients, visitors, physicians, and hospital staff.
• Promote professional growth through education and conferences.
• Follow patient safety policies and report safety concerns.
• Encourage patient participation in their own care.
