Job summary

Jobtome is seeking a Registered Nurse for Home Health Care in New Mexico. Provide direct nursing care to patients and families outside the hospital setting, coordinating home healthcare activities and ensuring adherence to hospital policies.

Qualifications

• Program Graduate (essential)
• Nationally Accredited Nursing Graduate (essential)
• RN licensure in NM or as allowed by NM reciprocal agreement (credential)
• CPR for Healthcare/BLS within 30 days (credential)
• 1 year of directly related experience (essential)

Responsibilities

• Assess patient's physical, psychological, social and environmental status; develop patient care plan tailored to needs assessment
• Provide nursing care and coordinates ancillary services to achieve treatment plan goals; supervise ancillary nursing staff inpatient care provision
• Administer medications and perform treatments for assigned patients, in accordance with medical and nursing protocols
• Report pertinent observations and reactions regarding changes in patient status to the appropriate team person
• Provide follow-up to ensure problem resolution; record observations and patient care activities
• Evaluate patient's response to approved plan of treatment; modify treatment plan as necessary
• Interpret agency philosophy and policies to patients, families, and other groups; utilize agency and community resources for client benefit
• Establish and maintain good rapport with patients, visitors, physicians and hospital staff
• Participate in educational programs and clinical in-services to advance professional growth
• Adhere to patient safety policies and proactively seek improvements to safety
