Job summary

Jobtome is seeking a Registered Nurse for a full-time Home Health Care role in Albuquerque, NM. You will provide direct nursing care to patients outside hospital settings, coordinate home health activities, and ensure compliance with policies.

Responsibilities include patient assessments, care planning, administering medications, and collaborating with the care team to meet treatment goals. Requires NM RN license and 1 year relevant experience.

Qualifications

• Program Graduate nursing degree completed
• Bachelor's degree in nursing or related field
• Nationally Accredited Nursing Graduate certification/credential
• RN license in NM or eligible by NM reciprocity

Responsibilities

• Provide direct nursing care to patients and families outside the hospital setting.
• Coordinate home healthcare activities with the care team.
• Ensure adherence to Hospital and Departmental Policies and Procedures.
• Assess patient physical, psychological, social and environmental status and develop care plan.
• Administer medications and treatments per medical and nursing protocols.
• Report changes in patient status to the appropriate team member.
• Follow up to ensure problem resolution and document care activities.
