Job summary

Jobtome seeks a dedicated Registered Nurse to provide direct patient care in home health settings. You will coordinate activities, administer medications, and collaborate with families, physicians and hospitals to support treatment plans across Neonate to Geriatric patients.

Responsibilities include assessing patient needs, developing care plans, ensuring safety, and supervising ancillary staff as needed. This full-time daytime role offers a sign-on and relocation benefits.

Qualifications

• Program Graduate
• Nationally Accredited Nursing Graduate
• 1 year directly related experience

Responsibilities

• Assess patient's physical, psychological, social and environmental status; develop patient care plan.
• Provide nursing care and coordinates services to achieve treatment goals.
• Administer medications and treatments per protocols.
• Report changes in patient status to the team.
• Provide follow-up to ensure problem resolution and record activities.
• Evaluate response to treatment and adjust plans as needed.
• Explain agency philosophy and policies to patients and families; use community resources.
• Establish rapport with patients, visitors, physicians and hospital staff.
• Encourage ongoing professional growth through education and workshops.
• Follow patient safety policies and protocols.
• Identify safety risks and report potential issues.
• Report medical errors and near misses promptly.
• Encourage patient involvement in care by asking questions.
• Ensure adherence to agency philosophy and care plans.
• Coordinate development and delivery of care with other teams.
