Job summary

Jobtome is seeking a Registered Nurse for Home Health Care in Albuquerque, NM. You will provide direct nursing care in patients\' homes, coordinate activities, and ensure adherence to policies. The role covers neonate to geriatric patients with a focus on safe, compassionate care.

Qualifications

• Graduate of nursing program.
• RN license in NM or eligible via reciprocal agreement by NM.
• CPR for Healthcare/BLS Prov or Prof Rescuers within 30 days.
• At least 1 year of direct patient care experience.
• Ability to work in home health care setting.

Responsibilities

• Assess patient's physical, psychological, social and environmental status; develop patient care plan tailored to needs assessment.
• Provide nursing care and coordinate ancillary services to achieve treatment plan goals; supervise staff.
• Administer medications and perform treatments per medical and nursing protocols.
• Report pertinent observations and changes in patient status to the appropriate team member.
• Provide follow-up to ensure problem resolution and record observations.
• Evaluate patient response to treatment and modify plan as needed.
• Interpret agency philosophy and policies to patients and families; utilize resources for client benefit.
• Establish and maintain rapport with patients, families, physicians and Hospital staff.
