Qualifications

• PA nursing license in good standing and graduation from an accredited nursing program.
• At least one year of hands-on medical experience with adult/geriatric populations in acute care, emergency, home health, hospice, ICU, med-surg or sub-acute settings.
• Ability to work independently and make real-time clinical decisions.

Responsibilities

• Triage and manage urgent clinical situations in the home setting.
• Perform comprehensive wound assessment, care, and documentation.
• Perform medication reconciliation, teaching, and safety monitoring.
• Maintain accurate point-of-care documentation and coordinate with the care team.
