Job summary

BAYADA Home Health Care is seeking a Full Time Registered Nurse (RN) to join our home health visit team in York, PA. You will provide skilled nursing and rehabilitative care in clients' homes, collaborating with a multi-disciplinary team and ensuring compassionate, high-quality home health services.

Qualifications include an active PA nursing license, graduation from an approved nursing program, and at least one year of hands-on medical experience in adult/geriatric settings.

Qualifications

A current nursing license in good standing in Pennsylvania.
Graduation from an accredited nursing program.
At least one year of hands-on medical experience within the past two years in adult/geriatric populations.

Responsibilities

Perform head-to-toe assessments and identify subtle changes in patient condition.
Apply experience with acute and post-acute care (CHF, COPD, wound care, IV therapy, post-surgical care).
Triage and manage urgent clinical situations in the home setting.
Perform medication reconciliation, teaching, and safety monitoring.
Conduct comprehensive wound assessment, care, and documentation.
