Job summary

Paycom - ATS is seeking a compassionate Registered Nurse to join our home health team in Shelby County, Ohio. You will manage patient care from admission through discharge, perform OASIS assessments, develop individualized plans, and collaborate with an interdisciplinary team to achieve optimal outcomes.

The role offers autonomy, flexibility, and meaningful patient relationships. Requirements include an Ohio RN license; OASIS experience is preferred; CPR/BLS; valid driver’s license; strong

Qualifications

• Current Ohio RN license is required.
• Experience with OASIS assessments is preferred but not required.
• CPR/BLS certification is required.
• Strong assessment, communication, organizational and time-management skills.

Responsibilities

• Performs Start of Care (SOC), Resumptions of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments.
• Develops individualized care plans and coordinates patient care throughout the episode.
• Provides skilled nursing services, patient education, medication management, and disease management.
• Collaborates with physicians, therapists, social workers and other team members.
• Completes OASIS documentation and clinical records per Medicare/state guidelines.
• Supports quality improvement initiatives and regulatory compliance.
