Job summary

Preble Home Health Services LLC in Eaton, OH is seeking a compassionate Registered Nurse to join our home health team. You will manage patient care from admission through discharge, perform OASIS assessments, and develop individualized care plans with an interdisciplinary team.

This role offers autonomy, flexibility, and the opportunity to build meaningful relationships with patients in their homes, while contributing to quality outcomes and continuing education.

Qualifications

• Active Ohio RN license is required.
• Home health experience and OASIS assessments preferred.
• Knowledge of Medicare home health regulations and documentation preferred.
• Current CPR and/or BLS certification.
• Valid driver's license with reliable transportation and proof of auto insurance.
• Strong assessment, communication, organizational and time-management skills.

Responsibilities

• Performs Start of Care (SOC), Resumption of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments.
• Conducts comprehensive nursing assessments and develop individualized plans of care.
• Coordinates and manages patient care throughout the episode of care.
• Provides skilled nursing services, patient education, medication management, and disease management.
• Collaborates with physicians, therapists, social workers and other members of the interdisciplinary team.
• Monitors patient progress and adjust care plans as needed.
• Completes accurate and timely OASIS documentation and clinical records in accordance with Medicare, state and agency guidelines.
• Participates in quality improvements initiatives and maintains compliance with regulatory standards.
