Paycom - ATS is seeking a compassionate Registered Nurse (RN) for Home Health in Greene County, Ohio and surrounding areas. The role covers patient care from admission through discharge, with OASIS assessments, care planning, and collaboration with a multidisciplinary team.

Responsibilities include SOC/ROC/recertification, disease management, and maintaining compliant documentation. Offers autonomy, flexibility, and ongoing professional development within a supportive team.

Qualifications

• Ohio RN license required.
• Experience in home health and OASIS preferred, but not required.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.
• Strong assessment, communication, and time-management skills.

Responsibilities

• Performs SOC, ROC, recertification, transfer, and discharge OASIS assessments.
• Develops individualized plans of care and coordinates episode care.
• Delivers skilled nursing services and patient education.
• Documents accurately per Medicare/state guidelines.
• Collaborates with interdisciplinary team and participates in quality initiatives.
