Paycom - ATS is seeking a compassionate Registered Nurse to join our home health team serving Shelby County, Ohio. You will manage patient care from admission through discharge, perform OASIS assessments, develop individualized care plans, and collaborate with an interdisciplinary team to achieve optimal outcomes.

This role offers autonomy and flexibility, with the chance to build meaningful relationships with patients in their homes while ensuring regulatory compliance and high‑quality care.

Qualifications

• Current Ohio RN license required.
• Home health experience and OASIS experience preferred.
• CPR/BLS certification required.
• Valid driver's license and reliable transportation.
• Strong assessment, communication, organization and time management skills.

Responsibilities

• Performs SOC, ROC, Recertification, Transfer, and Discharge OASIS assessments.
• Conducts comprehensive nursing assessments and develops individualized plans of care.
• Coordinates and manages patient care throughout the episode of care.
• Provides skilled nursing services, patient education, medication management, and disease management.
• Collaborates with physicians, therapists, social workers and other members of the interdisciplinary team.
• Monitors patient progress and adjusts care plans as needed.
• Completes accurate and timely OASIS documentation and clinical records in accordance with Medicare, state and agency guidelines.
• Participates in quality improvements initiatives and maintains compliance with regulatory standards.

In this role, you will manage patient care from admission through discharge, complete OASIS assessment, develop individualized care plans, and collaborate with an interdisciplinary team to achieve the best possible outcomes. • Conducts comprehensive nursing assessments and develop individualized plans of care
• Coordinates and manages patient care throughout the episode of care
• Provides skilled nursing services, patient education, medication management, and disease management
• Collaborates with physicians, therapists, social workers and other members of the interdisciplinary team
