Job summary

Paycom - ATS is seeking a compassionate Registered Nurse (RN) to join our home health team serving Greene County, Ohio and surrounding areas. You will manage patient care from admission through discharge, perform SOC/ROC/recertification, and develop individualized care plans.

Collaborate with physicians, therapists, social workers, and other team members; provide education, medication and disease management; ensure timely OASIS documentation and compliance with Medicare and agency guidelines.

Qualifications

• Current Ohio RN license
• Experience in home health and OASIS assessments preferred, but not required
• Knowledge of Medicare home health regulations and documentation requirements preferred, but not required
• Current CPR and/or BLS certification
• Valid driver's license, reliable transportation, and proof of auto insurance
• Strong assessment, communication, organizational and time management skills

Responsibilities

• Performs Start of Care (SOC), Resumptions of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments
• Conducts comprehensive nursing assessments and develop individualized plans of care
• Coordinates and manages patient care throughout the episode of care
• Provides skilled nursing services, patient education, medication management, and disease management
• Collaborates with physicians, therapists, social workers and other members of the interdisciplinary team
• Monitors patient progress and adjust care plans as needed
• Completes accurate and timely OASIS documentation and clinical records in accordance with Medicare, state and agency guidelines
• Participates in quality improvements initiatives and maintains compliance with regulatory standards
