Beacon Home Health in Jefferson City, TN seeks a compassionate Registered Nurse to join our on-the-road home health team. You will assess patients, coordinate care, and deliver nursing services in patients' homes across Hamblen/Jefferson County.

The ideal candidate has at least 1 year of clinical RN experience, an active RN license in TN, CPR certification, a valid driver's license and reliable vehicle. You will work under physician directives with strong support from The Pennant Group.

Qualifications

• 1 year of clinical RN experience.
• Current RN license in the state where services are rendered (TN).
• Current CPR certification.
• Driver's license, vehicle insurance, and access to a dependable vehicle.

A few position highlights include:Coordinate the total plan of care with other health care professionals involved in the care and strive to maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.Make the initial and/or comprehensive nursing evaluation visit, determine primary focus of care, develop the plan of care within state specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.Maintain and submit accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and patient’s response to home health care.Provide high quality clinical services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, from admit through discharge.THE MUST HAVES:One (1) year of clinical RN experience.Current RN license in the state where services are rendered.Current CPR certification, driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.Beacon Home Health, an affiliate of The Pennant Group (NASDAQ: PNTG), is the employer for this position as stated in the job posting.
