Job summary

Accurate Home Care seeks a Home Health RN Quality Care Specialist to enhance patient outcomes and documentation. The role emphasizes timely care interventions and interdisciplinary communication to improve satisfaction and ensure quality documentation.

The position is based in Otsego with remote options, offering 8-10 hours per week and growth potential. Requires MN RN licensure and strong English communication skills.

Qualifications

• Must be licensed as a Registered Nurse (RN) in Minnesota.
• Two years of home care experience, including OASIS data collection (preferred).
• Excellent oral and written communication skills.
• Ability to read, write and speak fluently in English.
• Working knowledge of Microsoft Office.
• Maintain confidentiality in accordance with applicable laws and rules.
• Provide four weeks' written notice prior to termination of employment.

Responsibilities

• Demonstrate critical thinking to advocate for patient safety, adherence to company policies, and conflict resolution.
• Review data entry in medical record system to ensure accuracy for billing and compliance.
• Conduct patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans as needed per manager direction.
• Ensure the development and implementation of the individualized plan of care.
• Assist with HHCAHPS survey analysis and implementation of quality improvement activities.
• Understand and comply with policy and procedures set forth by AHC.
• Monitors operations to ensure compliance with county, state and federal regulations affecting home health care.
