Job summary

Accurate Home Care is seeking a Home Health RN Quality Specialist to oversee care quality, documentation, and inter-disciplinary communication in a home health setting. This role includes patient assessments, care planning, and ensuring compliant documentation within Minnesota regulations.

The position offers a part-time schedule with 8–10 hours per week, a competitive hourly rate, and benefits including PTO accrual, 401K, and CEUs. RN licensure in Minnesota and OASIS familiarity are preferred.

Qualifications

• Must be licensed as a Registered Nurse (RN) in Minnesota.
• Two years of home care experience including OASIS data collection, preferred.
• Excellent oral and written communication skills.
• Working knowledge of Microsoft Office suite.

Responsibilities

• Demonstrate critical thinking to advocate for patient safety and adherence to policies.
• Review data entry in medical record system for billing and compliance accuracy.
• Conduct patient admission assessments, care planning, goal setting and care coordination.
• Develop and implement individualized plans of care.
• Support HHCAHPS survey analysis and quality improvement activities.
• Ensure compliance with county, state and federal regulations affecting home health care.

Position Summary:

Essential Job Duties and Responsibilities:

• Demonstrate critical thinking to advocate for patient safety, adherence to company policies, and conflict resolution.
• Review data entry in medical record system to ensure accuracy for billing and compliance.
• Conduct patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans as needed per manager direction.
• Ensure the development and implementation of the individualized plan of care.
• Assist with HHCAHPS survey analysis and implementation of quality improvement activities.
• Understand and comply with policy and procedures set forth by AHC.
• Monitors operations to ensure compliance with county, state and federal regulations affecting home health care.
• Additional tasks, as assigned by manager.

Education Requirements:

• Must be licensed as a Registered Nurse (RN) in Minnesota.

Qualifications Additional Requirements:

• Working knowledge of Microsoft Office suite.
• Excellent oral and written communication skills.
• Ability to read, write and speak fluently in English.
• Education and experience in the scope of services offered by the agency.
• Two years of home care experience including OASIS data collection, preferred.
• Maintain confidentiality in accordance with applicable laws and rules.
• Provide four week written notice prior to termination of employment.
