Job summary

Choice Health At Home is seeking a Home Health Field Registered Nurse (RN) for PRN duties in the Fort Myers area. The RN will conduct comprehensive in-home assessments, develop care plans, and coordinate with the care team to ensure quality service and patient safety.

The position requires an active RN license, CPR certification, and a valid driver’s license; HomeCare HomeBase experience is preferred. Join a team focused on compassionate, patient-centered home health care in Fort Myers, FL.

Qualifications

• RN license required.
• CPR certification required.
• Valid driver’s license required.
• Auto insurance required.
• 2+ years Home Health RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients in their homes.
• Develop, implement, and document individualized plans of care.
• Educate patients and families on self-care, nutrition, and disease management.
• Coordinate with physicians and interdisciplinary teams to ensure continuity of care.
• Provide clinical direction to Home Health Aide and LPN/LVN as needed.
• Record progress and communicate changes in patient status to the care team.

2+ years Home Health RN experience

HomeCare HomeBase experience preferred
