Job summary

UnitedHealth Group invites applications for a Home Health Registered Nurse (PRN) to deliver skilled, patient-centered care in patients' homes on an as-needed basis. Qualifications

• One year of clinical RN experience.
• Current RN license for the state you work in.
• Minimum six months home health experience.
• Current CPR certification.
• Valid driver’s license with reliable transportation.
• Able to assess patient status and age-specific needs.
• Familiar with computer use and online HR/training systems.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head-to-toe physical assessments.
• Checks vital signs and manages medications.
• Gives injections, IV therapy or tube feedings.
• Administers and maintains catheters.
• Completes documentation timely and accurately.
• Supervises LPNs and HHAs.
• Regularly communicates patient progress to the clinical manager, physician and care team.
