Job summary

Capitol Home Health in Texas is seeking a Registered Nurse (RN) focused on community health education and home care. The RN will plan, organize and direct home care services, leveraging community resources to meet individual and family needs within their homes and communities.

The role covers the Austin Metro Area and surrounding counties, with mileage reimbursement and other benefits. CPR certification and OASIS knowledge are preferred. PRN schedule offered to support patient care needs.

Qualifications

• RN license in Texas (unencumbered)
• Bachelor's degree in Nursing preferred
• 1–2 years of acute care experience
• Home health experience preferred
• CPR certification required with in-person skills eval
• Knowledge of OASIS documentation preferred

Responsibilities

• Plan, organize and direct home care services to meet patient needs
• Coordinate with community resources to support patients and families
• Monitor patient progress and adjust care plans as necessary
• Maintain clear written and verbal communication with care team and families
