Job summary

Capitol Home Health is seeking an experienced Registered Nurse to plan, organize and direct home care services in the San Antonio area. The RN will leverage community resources to tailor services for individuals and families in their homes and communities.

The role emphasizes community health education, care coordination, and adherence to CNAC/OASIS guidelines while providing compassionate patient care. This is a PRN position with mileage reimbursement and growth opportunities.

Qualifications

• Registered Nurse (RN) with current and unencumbered license to practice professional nursing in the state of employment.
• Bachelor's degree in Nursing from an accredited program, preferred.
• One to two years of recent acute care experience in an institutional setting.
• One year of home health care experience, preferred.
• Management experience not required, but direct supervisory experience of LVNs and Nurses Aides preferred.
• CPR certification current with in-person practical skills evaluation.
• Working knowledge of OASIS documentation preferred.
• Excellent observation, communication, problem solving, and basic math skills.

JOB DESCRIPTION SUMMARY

The Registered Nurse is experienced in nursing, with emphasis on community health education/experience. The RN is responsible for planning, organizing and directing home care services. The professional RN builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

QUALIFICATIONS

• One (1) year of home health care experience, preferred.


• Management experience not required, but direct supervisory experience of Licensed Vocational Nurses and Nurse Aides/Assistants, preferred.


• Possess and maintain valid CPR certification. Must have obtained in-person with a practical skills evaluation as part of the final test.


• Working knowledge of OASIS documentation requirements, preferred.


• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.


• Access to reliable transportation.
