Job summary

Paycom is seeking a compassionate registered nurse to join our Home Health Agency headquartered in Hyannis, MA. The role involves visiting patients at home on Cape Cod with a pay-per-visit model, averaging 2-3 patients per hour and earning $72–$108 per hour ($36 per 20-30 min visit).

Responsibilities include planning and coordinating care with providers, performing comprehensive assessments, and ensuring eligibility per payers.

Qualifications

• Graduate of an approved professional nursing program.
• Currently licensed in MA in good standing.
• Minimum of one year experience as a professional nurse, preferably in home care.
• Must have current CPR certification.
• Must pass criminal background check.
• Must be fluent in English.

Responsibilities

• Provide preventative and rehab nursing care.
• Coordinate plan of care with other healthcare professionals.
• Conduct initial/comprehensive nursing evaluation visits and develop the plan of care.
• Perform socio-psychological assessments and discharge planning.
• Ensure patients meet home health eligibility and medical necessity guidelines.
• Travel to patient residences for visits and assessments.
• Complete head-to-toe assessments and wound care when needed.
• Administer injections and manage medications as required.
• Participate in quality improvement and staff meetings.
