Job summary

Doctor's Choice Home Care & Hospice is seeking a Registered Nurse to provide intermittent in-home nursing care in the Beaumont, TX area. The role follows physician orders and the patient care plan, with involvement in patient and family education and coordination with the care team.

The RN will supervise LPNs and Home Health Aides, participate in QAPI activities, and travel as required. A current RN license, CPR certification, and valid driver's license are required for this per diem position.

Qualifications

• Currently licensed in the state(s) in which practicing.
• Two+ years of experience, home health preferred.
• CPR certification with American Heart Association or American Red Cross.
• Ability to exercise initiative and independent judgment.
• Strong communication skills, both oral and written, with excellent interpersonal skills.
• Must demonstrate good customer relations skills and a commitment to quality service.
• Must have a current valid driver's license, auto liability insurance and reliable transportation.
• Must be on call at least once per month.
• 100% travel may be required.

Responsibilities

• Administers skilled nursing care to patients in their place of residence on an intermittent basis.
• Follows physician orders and plan of care under the direction of the Director of Nurses.
• Participates in counseling and education of patients, caregivers, and families.
• Communicates with physicians and other health care practitioners involved in the plan of care.
• Provides services as ordered by the physician per the plan of care.
• Contributes to QAPI through ongoing interdisciplinary assessments of patients.
• Participates in development and revision of the plan of care with the patient and caregiver(s).
• Supervises care provided by LPNs and Home Health Aides.
