Job summary

Village Choice Healthcare Inc is seeking a compassionate Home Health Nurse to join our Beaumont, TX team. You will deliver high-quality, personalized care in patients' homes, thriving in an independent environment while making a meaningful difference in lives.

The role requires an active Texas RN/LVN license with home health experience preferred and strong clinical assessment, communication, and EHR skills.

Qualifications

• Active Texas RN or LVN license is required.
• Previous home health, hospice, or community nursing experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Valid driver's license and reliable transportation for home visits.
• Proficiency with electronic health records (EHR) and clinical documentation.
• Ability to work independently and manage a patient caseload efficiently.

Responsibilities

• Conduct in-home patient assessments, evaluations, and develop individualized care plans.
• Administer medications, wound care, IV therapy, and other skilled nursing treatments.
• Monitor and document patient progress, vital signs, and changes in condition.
• Coordinate care with physicians, therapists, and other healthcare team members.
• Educate patients and family members on disease management, medications, and home safety.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.
• Provide compassionate end-of-life and chronic disease management support as needed.
