Job summary

Beth Israel Lahey Health is seeking a registered nurse to plan, organize, and direct home care services within patients' homes and communities in Massachusetts. The role emphasizes community health education and adherence to home care regulations and payer guidelines.

You will initiate and revise care plans as patient needs change, administer medications, and supervise home health aides on a bi-weekly basis.

Qualifications

• Active Massachusetts RN license.
• Active BLS certification.
• Associate’s Degree or BSN from an accredited school (preferred).
• At least 1 year of direct care nursing experience; OASIS and/or EPIC experience preferred.

Responsibilities

• Plan, organize and direct home care services in patients' homes and communities.
• Initiate and revise the plan of care as patient status changes.
• Administer medications and treatments as prescribed by the physician.
• Instruct and supervise the home health aide care every two weeks.
• Identify discharge planning needs as part of care plan development.
• Maintain confidentiality in line with HIPAA guidelines.
