Job summary

Cobble Hill / YourChoice at Home Homecare in New York's Brooklyn and Queens communities is seeking compassionate Registered Nurses (RNs) for per diem, fee-for-service home health roles.

You will assess patients across Adult Geriatric, Women's Health, and Pediatric populations, develop care plans, administer treatments, and educate families. Reliable transportation and the ability to travel between boroughs are required.

Qualifications

• Active RN license in New York.
• Experience in Adult Geriatric, Women’s Health, and/or Pediatrics.
• Home health or community nursing experience is a plus.
• Strong clinical judgment, assessment, and communication skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation to travel throughout Brooklyn and Queens.

Responsibilities

• Provide skilled nursing care to patients in their homes across Adult Geriatric, Women's Health, and Pediatric populations.
• Perform comprehensive patient assessments and develop individualized care plans.
• Administer medications, treatments, and other physician-ordered interventions.
• Complete admissions, recertifications, and discharge visits.
• Educate patients and caregivers on diagnoses, medications, treatment plans, and disease management.
• Monitor patient progress and communicate changes in condition to the appropriate healthcare providers.
• Maintain accurate, timely documentation in accordance with agency, state, and federal regulations.
• Collaborate with physicians, therapists, social workers, and other interdisciplinary team members to ensure quality patient care.
