Job summary

Mederi Caretenders, part of the Optum family, is seeking an RN for home health to provide and coordinate nursing care in patients' homes across Florida. You will work with physicians, aides, and the care team to ensure ongoing, quality patient care in compliance with laws and agency policies.

The role emphasizes care planning, documentation, supervisory visits, and collaboration with families to meet patient needs. Competitive pay and benefits are provided.

Qualifications

• Current and unrestricted RN licensure in state of practice
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Preferred CPR Certification or ability to complete within 90 days of hire

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks according to state and federal regulations
• Provides required supervisory visits and ensures compliance with policy
• Performs comprehensive assessments (OASIS) including medication reconciliation
• Documents patient visits per policy and payer requirements, and coordinates with physicians and care team
• Communicates with physicians, patients, and family to ensure quality care and service excellence
• Participates in survey readiness and performance improvement activities
• Handles on-call and weekend rotations as needed
