Job summary

Mederi Caretenders, part of the UnitedHealth Group family, is hiring a Registered Nurse (RN) for home health in Florida. You will provide skilled nursing care in patients' homes, coordinate care plans with physicians and staff, and supervise aides and LPNs/LVNs to ensure high-quality outcomes.

Responsibilities include conducting OASIS assessments, documenting visits, and maintaining compliance with policies.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver’s License and access to reliable transportation.
• Preferred: CPR certification, 1+ year home health experience, ability to work independently, strong communication and organizational skills.

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with laws and agency policies.
• Coordinate total plan of care with other health care professionals and maintain continuity of patient care.
• Supervise home health aides and LPNs/LVNs, provide instruction, and assign tasks per regulations.
• Complete comprehensive assessments (OASIS) and ensure accurate medication reconciliation and documentation.
• Participate in on-call and weekend rotation as needed to meet patient needs.
