Job summary

Optum’s Elite Home Health division seeks a Registered Nurse to provide and direct nursing care in patients’ homes, coordinating the total plan of care with physicians and the care team while complying with laws and policies.

You will supervise home health aides, perform OASIS assessments, document visits, and ensure continuity of care, while communicating with patients and families to ensure quality outcomes.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• 1+ years of Home Health experience (preferred).

Responsibilities

• Initiates, develops, implements, and revises the plan of care with physician and health care professionals.
• Supervises home health aides and licensed nurses; provides instruction and assigns tasks per regulations.
• Provides required supervisory visits.
• Documents high-quality clinical care within plan of care and infection control standards.
• Completes comprehensive OASIS assessments including medication reconciliation.
• Documents visits per policy and payer requirements; adheres to LHC policy.
• Evaluates and determines home health eligibility and patient needs; develops plan of care with physician.
• Communicates with agency staff, physicians, patients and families to ensure quality care.
• Maintains timely and effective communication with physicians and family members.
• Assists Clinical/Executive Directors; participates in orientation and preceptorship.
• Participates in survey readiness and performance improvement; aims to reduce hospitalizations.
• Participates in on-call and weekend rotations; supports utilization management.
