Mass General Brigham Home Care, Inc. is seeking a part-time Registered Nurse (24 hours) to join the Minuteman team. The role covers Framingham and Waltham, with team support across Ashland, Fayville, Framingham, Hopkinton, Hudson, Southboro, Sudbury, Weston and Wayland.

RNs act as case managers, typically delivering 5–6 visits daily, while handling documentation, chart reviews and care coordination in a home-care setting.

Qualifications

• Graduate of an approved School of Nursing.
• Massachusetts license as Registered Nurse.
• Minimum of one year nursing experience in acute care; home care preferred.
• Travel within MGB Healthcare at Home geographic area.
• Ability to work with computer software and handheld devices.

Responsabilités

• Serve as case manager overseeing the full plan of care with an interdisciplinary team.
• typically complete 5-6 visits per day with time for documentation and coordination.
• Provide skilled nursing assessment, teaching, treatment and care coordination.
• Adhere to policies with timely electronic documentation.
