Job summary

DPS Home Health Agency Inc. is seeking caring, dependable Registered Nurses (RNs) with OASIS experience to provide skilled nursing care to patients in their homes across Palm Beach Gardens, FL and surrounding communities.

You will complete comprehensive patient assessments, develop and implement individualized care plans, administer treatments, educate patients and caregivers, and coordinate with physicians to support patient outcomes.

Qualifications

• Active Florida RN license.
• Current CPR/BLS certification.
• OASIS experience.
• Excellent assessment, communication, and organizational skills.
• Valid Florida driver’s license and reliable transportation.
• Ability to work independently while providing compassionate, patient-centered care.

Responsibilities

• Conduct comprehensive patient assessments, including OASIS documentation for Medicare-certified episodes.
• Develop, implement, and update individualized care plans.
• Administer medications and treatments as ordered by the physician.
• Monitor patient progress and communicate changes in condition.
• Provide wound care and other skilled nursing interventions.
• Educate patients, family members, and caregivers on disease management, medications, and home care.
• Maintain accurate and timely clinical documentation.
• Coordinate care with physicians, therapists, and other members of the healthcare team.
• Ensure compliance with Medicare, state, and company policies.

About The Role

As a Home Health Registered Nurse, you will complete comprehensive patient assessments, including OASIS documentation for Medicare-certified episodes. You will develop and implement individualized care plans, administer treatments, educate patients and caregivers, and collaborate with physicians and other healthcare professionals to support patient outcomes.

Preferred Experience

At least one year of nursing experience is preferred. Home health experience is an asset but is not required.

Preference will be given to candidates with experience in one or more of the following:

• Home health.
• Wound care.
• IV therapy.
• Chronic disease management.
• Diabetes education.
• Tracheostomy care.
• Gastrostomy tube (G-tube) care.
