Job summary

DPS Home Health Agency Inc. is seeking a compassionate Home Health Registered Nurse (RN) with OASIS experience to provide skilled nursing care in patients' homes around Riveria Beach, FL 33404 and surrounding communities.

You will perform comprehensive patient assessments, develop and implement individualized care plans, administer treatments, educate patients and caregivers, and collaborate with physicians to support outcomes.

Qualifications

• Active Florida RN license required.
• Current CPR/BLS certification required.
• OASIS experience preferred.
• Excellent assessment and communication skills.
• Valid Florida driver's license and reliable transportation.
• Ability to work independently while providing compassionate care.

Responsibilities

• Conduct comprehensive patient assessments with OASIS documentation.
• Develop, implement, and update individualized care plans.
• Administer medications and treatments as ordered.
• Monitor progress and communicate changes to the team.
• Provide wound care and other skilled nursing interventions.
• Educate patients and caregivers on disease management and home care.
• Coordinate care with physicians and other healthcare professionals.

About The Role

As a Home Health Registered Nurse, you will complete comprehensive patient assessments, including OASIS documentation for Medicare-certified episodes. You will develop and implement individualized care plans, administer treatments, educate patients and caregivers, and collaborate with physicians and other healthcare professionals to support patient outcomes.

Qualifications

• Active Florida Registered Nurse (RN) license.
• Current CPR/BLS certification.
• OASIS experience.
• Excellent assessment, communication, and organizational skills.
• Valid Florida driver’s license and reliable transportation.
• Ability to work independently while providing compassionate, patient-centered care.

Preferred Experience

At least one year of nursing experience is preferred. Home health experience is an asset but is not required.

Preference will be given to candidates with experience in one or more of the following:

• Home health.
• Wound care.
• IV therapy.
• Chronic disease management.
• Diabetes education.
• Tracheostomy care.
• Gastrostomy tube (G-tube) care.
