Job summary

Qualifications

• Active RN license in Ohio.
• Current BLS certification through the American Heart Association (AHA).
• BSN degree preferred.
• Strong critical thinking and proactive documentation abilities.
• Leadership experience with a multidisciplinary team.
• Excellent communication and time management skills.

Responsibilities

• Act as a clinical leader to support positive patient outcomes and care experiences.
• Review and optimize home care plans to meet patient needs safely and efficiently.
• Manage a multidisciplinary team to deliver safe, compassionate home care.
• Monitor clinical documentation, perform audits, and implement corrective actions.
• Assist with onboarding, orientation, and ongoing staff training.
• Ensure home health processes comply with OSHA, ODH, BOP, and TJC.
• Drive cost containment by using facility resources appropriately.
• Maintain confidentiality of PHI/HIPAA and patient information.
• Participate in facility training initiatives and other leadership duties.
