Elara Caring is seeking a Registered Nurse to support home health across patient homes. The role emphasizes high-quality, compassionate care delivered in the patient’s home setting, with responsibilities including assessments, care planning, and ongoing education for patients and families.

Applicants should hold a current RN license and have 1+ year of clinical nursing experience. Home health experience is preferred, but not required; lifting up to 50 pounds and reliable transportation are

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate/ Diploma/ Associate or Bachelor).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred but not required.
• Ability to lift, push, pull and transfer patients up to 50 pounds and travel between homes.

Responsibilities

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.
