Job summary

Elara Caring is hiring a Registered Nurse Home Health to provide high-quality, in-home care to patients across our service area. You will assess, plan, implement, and evaluate care plans during visits, coordinating with an interdisciplinary team to support each patient’s goals.

This role requires an active state RN license, at least 1 year of clinical nursing experience, and willingness to travel up to 50% within a designated area; compassionate teamwork and patient interaction are essential.

Qualifications

• Ability to travel within branch/office coverage area

Responsibilities

• Provides in-home nursing care to patients.
• Assesses, plans, implements, and evaluates care plans during visits.
• Communicates findings and changes to Clinical Supervisor and team.
• Maintains patient privacy and HIPAA compliance.
• Revises plan of care as patient needs evolve.
• Reports and collaborates with physician and facility staff.
