Job summary

Elite Home Health, a part of LHC Group, is seeking a Registered Nurse for home health services. You will provide and direct nursing care in patients' homes as prescribed by physicians, coordinating care with the team and ensuring compliance with regulations.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current driver's license and access to dependable transportation or vehicle.

Responsibilities

• Initiates and revises the plan of care with the physician and care team.
• Supervises home health aides and licensed nurses per state/federal regs.
• Completes comprehensive assessments (OASIS) and medication reconciliation.
• Documents visits per policy and payer requirements.
• Participates in on-call and weekend rotations and survey readiness.
