Job summary

Idaho Home Health, part of LHC Group, is hiring a registered nurse to deliver nursing care in patients' homes under physician orders, coordinating care with the broader healthcare team.

You will lead care planning, supervise aides and licensed nurses, perform OASIS assessments, document visits, and collaborate with families to ensure safe, high-quality home-based care. This role supports on-call rotations and requires strong clinical and communication skills.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Valid driver's license and access to reliable transportation.
• CPR certification preferred or able to obtain within 90 days of hire.
• 1+ years of Home Health experience.
• Ability to work independently.
• Strong communication and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed nurses; assigns tasks per regulations.
• Provides required supervisory visits.
• Performs comprehensive assessments (OASIS) including medication reconciliation.
• Documents patient visits per policy and payer requirements.
• Coordinates care with physicians, patients, families, and community resources.
• Participates in on-call and weekend rotation.
