Qualifications

• Current and unrestricted RN licensure in state of practice
• Current Driver's License and access to a dependable vehicle
• Preferred CPR certification or ability to complete within 90 days of hire
• 1+ years of Home Health experience

Responsibilities

• Initiates and revises plan of care with physician and team
• Supervises home health aides and licensed nurses per regulations
• Provides high-quality clinical services and infection control per plan of care
• Completes comprehensive assessments (OASIS) accurately and timely
• Participates in on-call and weekend rotation as needed
