Job summary

Providence at Home with Compassus is seeking a Home Health Registered Nurse to deliver expert care in patients' homes. You will perform comprehensive assessments, develop individualized care plans, and provide interventions aligned with physician-ordered Plans of Care, ensuring safety and quality in a home-based setting.

The role emphasizes teamwork with an interdisciplinary group, adherence to nursing standards, ongoing education, and support for orientation of new staff.

Qualifications

• Associate or Bachelor's degree in Nursing from an accredited program.
• Preferred 1 year of recent experience in home health, acute care, or skilled nursing facility.

Responsibilities

• Conduct comprehensive patient assessments upon admission and ongoing reassessment.
• Develop, document, and implement individualized plans of care with interdisciplinary team.
• Provide safe, quality nursing care following the Plan of Care and Nurse Practice Act.
• Mentor and orient new staff and students as needed.
