Job summary

Providence at Home with Compassus is hiring a Home Health Registered Nurse (RN) to provide expert nursing care in patients’ homes. You will perform comprehensive assessments, develop care plans, and deliver outcomes-driven interventions within the physician-ordered Plan of Care.

This role emphasizes teamwork, documentation, and patient-family-centered care. The position requires an RN license, BLS certification, and 1 year of relevant experience.

Qualifications

• Current, unencumbered RN license in the state of employment.
• BLS certification from American Heart Association is required.
• Preferred experience in home health, acute care, or skilled nursing facility.

Responsibilities

• Conduct comprehensive patient assessments upon admission.
• Develop and implement individualized plans of care.
• Collaborate with patients, families, and healthcare team to monitor progress and adjust care plans.
• Document care activities accurately and in a timely manner.
• Participate in ongoing staff education and orientation activities.
