Job summary

UNC Health in North Carolina is seeking a dedicated Home Health Nurse who coordinates and delivers patient care in the home setting under physician guidance. You will apply the nursing process, document in EMR, and collaborate with an interdisciplinary team to build individualized care plans.

The role requires a valid RN license in NC or a compact state, at least 1 year of hospital RN experience, BLS, and a valid driver's license. A hybrid work arrangement is offered.

Qualifications

• RN license in NC or a compact state by first day of clinical care.
• 1 year of practice as an RN in a hospital setting.
• Valid driver's license and access to a vehicle; transit if needed.
• BLS for Healthcare Provider certification.

Responsibilities

• Organizes and delivers nursing care in the home setting.
• Collaborates with the interdisciplinary care team to develop care plans.
• Synthesizes data to set priorities ensuring patient independence.
• Defines independent schedule with scheduling support.
• Communicates with patient/family to update the plan of care.
• Develops clinical and operational excellence through education and training.
• Demonstrates proper performance of skills per orientation/skills checklist.
