Job summary

Talin Home Healthcare LLC is seeking multiple compassionate Home Health Nurses to serve patients across the Northern Virginia counties. This role offers independence, meaningful patient relationships, and the chance to provide high-quality, in-home care.

Qualified candidates will hold an active Virginia RN license, with Oasis SOC experience preferred. Travel throughout the NOVA region is required, and a minimum of two days per week, including a Monday or Tuesday, is expected.

Qualifications

• Active RN license in Virginia.
• Oasis SOC Experience preferred.
• Home health, med-surg, or community nursing experience preferred.
• Excellent communication and patient education abilities.

Responsibilities

• Provide skilled nursing care to patients in their homes across the Northern Virginia area.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications, wound care, IV therapy, and other clinical treatments as ordered.
• Monitor and document patient progress, vital signs, and changes in condition.
• Educate patients and family members on disease management, medications, and home care techniques.
• Collaborate with physicians, therapists, and care team members to ensure coordinated patient care.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.
