Job summary

Talin Home Healthcare is seeking compassionate Home Health Nurses to serve patients across the Northern Virginia counties. Join a team that supports independent practice, builds real patient connections, and delivers high‑quality at‑home care.

You will assess patients, plan care, administer medications, and educate families while coordinating with physicians and therapists. A Virginia RN license and reliable transportation are required for a flexible 3‑day workweek.

Qualifications

• Active RN license in Virginia.
• OASIS and SOC experience preferred.
• Previous home health, med-surg, or community nursing experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Reliable transportation and valid driver’s license for travel in NOVA region.
• Commit to working a minimum of 3 days a week Mon/Wed/Fri.

Responsibilities

• Provide skilled nursing care to patients in their homes across the Northern Virginia area.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications, wound care, IV therapy, and other clinical treatments as ordered.
• Monitor and document patient progress, vital signs, and changes in condition.
• Educate patients and family members on disease management, medications, and home care techniques.
• Collaborate with physicians, therapists, and care team members to ensure coordinated patient care.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.
