Job summary

Talin Home Healthcare LLC is seeking compassionate Home Health Nurses to serve patients across the Northern Virginia counties. You will deliver high-quality, personalized care in patients’ homes, building real connections and supporting families.

Qualifications

• Active registered nurse license in Virginia.
• Oasis and SOC experience preferred.
• Home health, med-surg, or community nursing experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Reliable transportation and valid driver's license for NOVA travel.
• Ability to work independently with a flexible caseload.
• Compassionate, patient-centered approach to care.
• Commitment to working a minimum of 3 days a week: Mon/Wed/Fri.

Responsibilities

• Provide skilled nursing care to home patients across the Northern Virginia area.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications, wound care, IV therapy, and other treatments as ordered.
• Monitor patient progress, vital signs, and changes in condition.
• Educate patients and families on disease management, medications, and home care techniques.
• Collaborate with physicians, therapists, and care team members for coordinated care.
• Maintain accurate clinical documentation in compliance with regulatory standards.
