Job summary

Talin Home Healthcare LLC in Sterling, VA is seeking multiple compassionate Home Health Nurses to provide skilled nursing care in patients’ homes across the NOVA region. You’ll work independently, build meaningful connections, and deliver personalized care to help patients live safely at home.

Requirements include an active Virginia RN license, prior home health or med-surg nursing, strong assessment and communication skills, reliable transportation, and a commitment of at least 2 days per week

Qualifications

Active RN license in Virginia required.
Experience in home health, med-surg, or community nursing preferred.
Strong clinical assessment and critical thinking skills.
Excellent communication and patient education abilities.
Reliable transportation and travel across NOVA region.
Commit to at least 2 days per week, with 1 day Monday or Tuesday.

Responsibilities

Provide skilled nursing care to patients in their homes across the NOVA area.
Conduct comprehensive patient assessments and develop individualized care plans.
Administer medications, wound care, IV therapy, and other treatments as ordered.
Monitor and document patient progress, vital signs, and changes in condition.
Educate patients and families on disease management, medications, and home care techniques.
Collaborate with physicians, therapists, and care team for coordinated care.
Maintain accurate clinical documentation per regulatory standards.
