Job summary

Pathways Healthcare is seeking a dedicated nurse to join our home health and hospice team in Virginia. You will assess patients, develop and revise care plans, and coordinate with physicians to deliver high-quality, patient-centered care in the home setting.

Requirements include 2+ years of nursing experience (geriatrics preferred), a Virginia nursing license, BLS CPR, and a valid driver's license. Proficiency with electronic medical records and strong communication are essential.

Qualifications

• 2+ years' nursing experience, preferably in geriatrics.
• Nursing degree from an accredited institution and an active Virginia license.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient status and writes plan of care.
• Regularly re-evaluates patient and family needs.
• Develops care plans based on nursing diagnoses and goals.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed.
• Maintains accurate clinical notes and documents changes.
• Communicates with physicians about patient needs and orders.
