Job summary

Pathways Healthcare seeks a dedicated Registered Nurse (RN) for our home health and hospice program in Virginia. You will assess patients, initiate and revise care plans, and coordinate with physicians to ensure compassionate, patient-centered care.

Applicants must hold an active Virginia RN license, have 2+ years of nursing experience (geriatrics preferred), and be proficient with EMR systems. A valid driver's license and reliable vehicle are required for daily visits.

Qualifications

2+ years nursing experience, preferably in geriatrics.
Active nursing license in Virginia.
Proficient with electronic medical records.
Excellent bedside manner and communication skills.
Working knowledge of health care laws and regulations.
Maintain current BLS CPR certification.
Possess a valid driver's license and own a reliable vehicle.

Responsibilities

Assesses and evaluates patient status; writes and initiates plan of care.
Regularly re-evaluates patient and family needs; revises the plan as necessary.
Develops a care plan with goals based on nursing diagnosis and involves patient/family.
Administers medications and treatments as prescribed by the physician.
Completes and maintains accurate clinical notes and documents changes.
Communicates with the physician regarding patient needs and orders.
Provides a safe environment for the patient.
Attends interdisciplinary group meetings.
Participates in on-call duties.
Ensures arrangements for equipment and services are available.
Supervises ancillary personnel and delegates responsibilities as required.
Pursues ongoing professional development and education.
Fulfills case assignments and engages in quality improvement activities.
