Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they practice.
• At least one year experience as a licensed professional.
• Basic technology skills, including operation of a mobile device.
• Medicare home health or hospice experience preferred.
• Experience as a licensed professional may be deferred with regional leadership approval.

Responsibilities

• Administer skilled care to patients per physician orders and plan of care.
• Teach the patient, family, and health care team members.
• Document care and communicate with the health care team.
